When a patient reaches their goals or the benefit runs out but is not fully back to strength, we continue the work under your framework. We read your discharge summary, respect stated precautions and weight-bearing limits, progress conservatively, and send you a written report after every visit with objective measures — chair-rise counts, gait observations, balance holds and any reported falls. If something changes for the worse, you hear from us before the next visit rather than at the next appointment. No fee to refer, and we never present ourselves as providing physical therapy.

Home health

Home healthcare providers & agencies

We add specialist fitness capacity alongside the care your team already delivers, in the client's own home and on a schedule that works around your visits. Aides can focus on care tasks while a certified senior-fitness trainer handles progressive strength and balance work. Your care manager gets read-only access to visit reports, fall logs and attendance, so documentation stays aligned and nothing has to be chased by phone.

Concierge medicine

Concierge & primary-care physicians

An accountable exercise partner for patients you would otherwise advise, vaguely, to "stay active". We complete a structured baseline at the first visit, repeat it at set intervals, and give you fall-risk trends, adherence data and medication-related steadiness observations you can act on in a ten-minute appointment. Useful evidence for annual wellness visits, fall-risk documentation and quality measures.

Senior living communities

Independent, assisted living & memory care

Recurring one-to-one and small-group training inside your building, with no added headcount, no equipment purchase and no billing relationship for your team to manage. Residents and families contract with us directly; you receive participation reporting, incident and fall notes, and monthly wellness summaries you can show prospects and families. Insurance, background checks and credential verification are our responsibility and are documented for your files.

Skilled nursing & rehab

Skilled nursing and short-stay rehab

A discharge destination for residents going home who still need supervised progression. We can start within days of discharge, in the home, so the gains made in your therapy gym do not disappear in the first month. Discharge measures are used as our baseline, and re-measurement at six weeks tells you — and the resident's family — whether the transition held.

Case managers & elder law

Care managers, elder-law and trust professionals

For clients whose independence at home is being actively managed or funded, we provide documented, invoiced services with clear session records suitable for trust accounting, long-term-care claims and family reporting. Aging life care professionals get their own read-only login covering scheduling, attendance, fall logs and progress trends across every client they oversee.

What happens after you refer

1. We call within one business day. A real person speaks with the patient or family and books a free assessment visit.

2. We work within your plan of care. Certified senior trainers start at the level your notes allow and progress only when movement looks steady.

3. You see whether gains hold. Every visit produces a plain-language report with balance, strength and mobility measures. With the patient’s permission we share it with you.
